Care Management of Frail Older Adults

What is Care Management?

Care Management assists older adults who have complex care needs by identifying, accessing, and coordinating services necessary for an individual to remain safely at home. This coordinated care function includes case finding, assessment, care plan development and implementation, negotiation, monitoring, and advocacy.

Who qualifies for Care Management?

Individuals who are 60 years of age or older with complex care needs that affect their activities of daily living.

How do I access the Service?

Care Management is offered by local service providers supported by the Home and Community Care Block Grant. A list of local providers can be found by clicking the link below.
