Home Care Fee Schedule
Licensure fees that accompany an initial licensure application will be $560.00. Licensure fees that accompany an application for a change of ownership will be $510.00. A nonrefundable annual licensure fee will be in the amount of $510.00.
Hospice Fee Schedule
Licensure fees that accompany an initial licensure application for hospice will be $450.00. A nonrefundable annual licensure fee will be in the amount of $400.
Please note: All license fees are nonrefundable regardless of when a license is finally issued.
