Community Care of North Carolina/Carolina ACCESS (Managed Care)

CCNC/CA is North Carolina’s Medicaid managed care program. It provides you with a medical home and a primary care provider (PCP) who will coordinate your medical care.

As a CCNC/CA member, you are eligible for all the services that Medicaid covers. Being a member also has the following advantages:

• You can choose a medical home with a primary doctor. A medical home can be chosen for each family member. Your local County Department of Social Services (DSS) office has a complete list of participating doctors.  If you do not choose a medical home, you will be automatically assigned to one. 
• You can call your primary doctor day or night for medical advice. Check your Medicaid ID card for your doctor’s daytime and after-hours phone numbers.
• You may have a care manager who can help you understand and manage your health care. A care manager can show you how to stay healthy.  Ask your doctor for more information about obtaining a care manager and how a care manager can help you.
• You receive regular sick care and well care at your medical home. These services include:
  • Yearly physical check-ups
  • Immunizations
  • Pap smears
  • Prescriptions for medicines when necessary
• Your primary doctor will make referrals to other doctors if you need special care not provided in your primary doctor’s office. Examples of special services may be mammograms or other tests needed to diagnosis a health problem.

Who is eligible

Most recipients of Medicaid are required to enroll with CCNC/CA when you first enroll with Medicaid.

How to Enroll with CCNC/CA

The enrollment is an automatic part of the Medicaid application process.

Exemptions

Most people benefit from being a CCNC/CA member. There may be a medical reason in which being a member is not as beneficial as remaining in regular Medicaid.

Your Medicaid program may require you to also enroll as a member of CCNC/CA.  If you think you should not be enrolled for medical reasons, contact your caseworker at local County Department of Social Services (DSS) office and ask for a medical exemption request form and instructions. This form should be completed by your doctor.  A nurse in the Division of Medical Assistance Managed Care section reviews and approves or denies the request.  If your request is denied, call your caseworker to choose a medical home. If you do not call to choose a medical home, you will be automatically assigned to one.

Complaints

As a CCNC/CA member, you have a right to make a complaint if you think your primary doctor has not provided good care. You may make a complaint for any of the following reasons:

• You think you have to wait too long to get care.
• You think you received bad care.
• You are not able to contact the primary doctor’s office during the day or after hours.
• Your doctor or someone in the office said something inappropriate to you.
• Your doctor or someone in the office has treated you inappropriately (physical or sexual contact).
• You suspect your doctor or someone in the office has used alcohol or drugs.

All complaints are confidential unless you agree for it to be shared with the doctor.
