All Free-Standing and Hospital Based Nursing Facilities with 2008 cost report year ends on or before September 30, 2008 are required to complete and submit the North Carolina Medicaid Capital Data Survey 2008 to DMA no later than Wednesday, December 31, 2008.
INSTRUCTIONS
All Nursing Facilities:
Please review your facility(ies) capital data survey information. Please verify that the total number of beds, initial year of construction, and number of initial nursing home beds are correct. If your total nursing home beds are more than your initial nursing home beds, please verify the bed additions on the aging schedule, along with the initial beds, add up to the total nursing home beds. Verify all entries on the aging schedule are correct for type, year, and amount. Also, verify all entires, beds and amounts, are ONLY for your nursing beds and no information submitted is for non-Nursing beds, such as Rest Home beds or ACH beds.
If you find any discrepency, please complete a new Capital Data Survey (CDS) with ALL entires included as they are to appear on the aging schedule, along with total nursing facility beds, initial year of construction and initial nursing facility beds. Any entry that is not on a revised CDS that may be submitted will be deleted from the schedule. All revisions are due by April 15, 2009. Please foward to:

All Free-Standing and Hospital Based Nursing Facilities with 2007 cost report year ends on or before September 30, 2007 are required to complete and submit the North Carolina Medicaid Capital Data Survey 2007 to DMA no later than Monday, December 31, 2007.
