Your caseworker will mail you a notice to tell you if your application for North Carolina Health Choice was approved.
You will receive one identification (ID) card in the mail when you enroll your child in NC Health Choice. The ID card is valid for 12 months.
You should sign the ID card upon receipt. You must take the ID card with you every time you take your child to a doctor, clinic, hospital or other health care professional for medical care or to the drug store why you buy a prescription drug. Your NCHC card is your proof that you have coverage, just like insurance.
If you or your family members do not show your card to the person treating you or your family member, the person will not know you are covered by NCHC. You may then be responsible for paying the full cost of the treatment or prescription drug.
If you lose your ID card, call Customer Service at 1-800-422-4658, Monday through Friday, 8 a.m. to 5 p.m. and request a replacement card. You can also visit and register with the Blue Cross Blue Shield of North Carolina - My Member Services website to order a new ID card.
