Hospital-Based Nursing Facility Cost Reporting Software and User Documents

If you must report financial information to DMA for a Hospital-Based Nursing Facility that provides Long Term Skilled Nursing Care (SNC) and/or Intermediate Nursing Care (INC) then select the cost reporting software, user manual, and Instructions for Cost Report Preparation document you need from the following list. 
  • Choose the appropriate Fiscal Year End (FYE) and note the Due Date for the cost report to be received by DMA. 
  • You have the option to download the software user manual or refer to the document on-line as needed using the appropriate links below. 
  • In addition, you will need to download the Instructions for Cost Report Preparation document as it contains specific DMA requirements that must be followed when preparing the cost reporting forms. 
  • The User Manual and the Instructions for Cost Report Preparation documents are in portable document format (.pdf). The free Adobe Acrobat Reader is required to view or print PDF publications. If you do not have Acrobat Reader, get it here! 

IMPORTANT NOTICE: All Hospital-based Nursing Facilities with 2008 cost report year ending on or before September 30, 2008 are required to complete and submit the North Carolina Medicaid Capital Data Survey 2008 to DMA with their cost report. See Guidance for Cost Report Preparation.
