Download the Cost Reporting Software and User Documents You Need

If you are a Home Office or Related Party Organization supporting Intermediate Care Facilities / Mental Retardation Facilities (ICF/MR) and are required to report financial information to DMA:

• Select the cost reporting software you need from the following list. Choose the appropriate Fiscal Year End (FYE) and note the Date Due for the cost report to be received by DMA.
• The Home Office Cost Statement Instructions Manual is available for users to download or refer to on-line as needed.
• In addition, you will need to download the Guidance for Cost Report Preparation document as it contains specific DMA requirements that must be followed when preparing the cost reporting forms.
• The Instructions Manual and the Guidance for Cost Report Preparation documents are in portable document format (.pdf). The free Adobe Acrobat Reader is required to view or print PDF publications, If you do not have Acrobat Reader, get it here!
