Home Office Cost Reporting Software
and User Documents
If you are a Home Office or Related Party Organization supporting
Nursing Facilities providing Long Term Skilled Nursing Care
(SNC) and/or Intermediate Nursing Care (INC) and are required to report
financial information to DMA.
- Select the cost reporting software you need from below. Choose the
appropriate Fiscal Year End (FYE) and note the Date Due for the cost
report to be received by DMA.
- You have the option to download the Home Office Cost Statement Instructions
Manual or refer to the document on-line as needed using the appropriate
links below.
- In addition, you will need to download the Guidance for Cost Report
Preparation document as it contains specific DMA requirements that must
be followed when preparing the cost reporting forms.
- The User Manual and the Guidance for Cost Report Preparation documents
are in portable document format (.pdf). The free Adobe Acrobat Reader
is required to view or print PDF publications. If you do not have Acrobat
Reader, get
it here!
IMPORTANT NOTICE: The software has provisions for
the new National Provider Identifier (NPI) numbers. See Guidance for
Cost Report Preparation for more information on proper usage of NPI and
Medicaid provider numbers.
