A Hospital-Based Nursing Facility providing Long Term (INC) and/or (SNC) Level of Care
An Intermediate Care Facility/Mental Retardation Facility (ICF/MR) providing Long Term Care
A Nursing Facility providing Long Term Skilled Nursing Care (SNC) and/or Intermediate Nursing Care (INC)
A Home Office or Related Party Organization supporting Long Term Care ICF/MR Facilities
A Home Office or Related Party Organization supporting Long Term Care Nursing Facilities
