What it means to you

This page is designed to give you more information about how Innovations Plus works and how it will affect you.

There are three main parts to Innovations Plus that will affect you:

1. You will have your support needs assessed using the Supports Intensity Scale.
2. You will be given an Individualized Budget Amount that is based on the results of the SIS interview and other factors.
3. You will work with your Care Coordinator to develop an Individual Support Plan to receive services. You can use the information from the SIS results to help develop your plan. The plan will also be related to your Individualized Budget Amount.
