Proposed Medicaid Clinical Coverage Policies
As a result of the Case Management Steering Committee's work, four work groups have been established to assist in the development and implementation of consolidated case management services.
The Service Definition Work Group will define the criteria for case management services.

Community Alternatives Program (CAP) Waivers Standardization Work Group
The Community Alternatives Program (CAP) Waivers Standardization Work Group will review CAP Waiver legislation and policies and, where possible, align program policies.
The HCBS 3.5 Waiver Application Instructions, Technical Guide and Review Criteria can be accessed from the CMS Waiver Application website .
When you go to this page, you will see a column of text on the left of the screen under the header "Welcome." Scroll down to the section titled "1915(c) Application under the heading titled "Links and Downloads." Click on "1915(c) Waiver Application and Accompanying Materials" and follow the instructions for downloading these documents.
