Targeted Case Management for individuals with intellectual and developmental disabilities or individuals who are at risk for developmental disabilities or delays manifested before age 22, is a service available statewide that assists recipients in gaining access to needed services described in the North Carolina State Medicaid Plan, as well as needed medical, social, educational, and other services.
