Adoption Status Request

If you are an adoptive parent and have completed an adoption proceeding through the Final Decree but have not yet received notification from the State Division of Social Services that you can now request your child's new birth certificate, we suggest you fill out the following information and E-mail it to: Tammy.T.Johnson@dhhs.nc.gov

Due to the high volume of adoptions, we ask that you allow at least 3 months from the date of your Final Decree before requesting the status. Every effort will then be made to process your child's adoption in a timely manner.
