The following chart describes the Crisis Intervention Eligibility Requirements.

PLEASE NOTE: In reference to the "WHAT'S NEEDED" column on the chart., if we are unable to establish eligibility for assistance, additional information may be required.

Data Requirement	What's Needed	How Often
Identity	Contact with someone knowledgable of your situation, ID	At application or if questionable
Address	Lease, rental agreement, contact with someone knowledgable of your situation	At application
Citizenship/Alien Status	Verification of citizenship/alien status via Immigration and Naturalization Service	At application, client statement
Social Security Number	Valid social security card or number	At application
Income: Wages/Self-Employment Public Benefits/ Private Retirement Benefits/ Trust, etc.	Wage stubs, tax forms, contacting employer, Document from provider, award letter	At application, review, change in situation
Heating or Cooling Expenses	Copy of bills, receipts, statements,etc	At application
