Voluntary Insurance Coverage from Pierce Insurance and American Heritage Life

Through the facilities of Pierce Insurance of Farmville, NC, American Heritage Life is privileged to make available two of its voluntary insurance products to the employees of DHHS. They are voluntary accident insurance and voluntary disability income insurance. The voluntary accident insurance program provides benefits for covered off-the-job accidents. The disability income plan pays if you are unable to work because of a covered accident or sickness.

Below is an enrollment form and a proof of good health statement. If you are interested in applying for this disability income insurance, please print these forms, complete them, and return them to your Personnel Coordinator. They will then be forwarded to Pierce Insurance for processing.

If you have a disability claim and are covered under the disability income insurance program, below are instructions and a claim form. Please see your Personnel Coordinator for assistance in completing them, and in submitting them to American Heritage Life for processing.
