Endorsement Information for Providers

Archived Provider Endorsement Information

Below are two forms and a question/answer document that LME/MCOs and Providers may start using as part of the endorsement transition. The State Plan Amendment has not been approved as of this date.

• Question and Answer Document
• Notification of Credentialing Action Letter (which replaces the NEA)
• Agency Provider Additional Services Application

Endorsement Eliminated:

DMH, DMA, NC Council representatives of LME/MCOs, and the AG's office have been working on a transition plan for endorsement activity to be replaced by credentialing.  Once DMA has submitted a revision of the SPA regarding endorsement, an official announcement with guidance regarding the credentialing process will be posted on DMH, DMA, and the NC Council's WebPages.
