Guide to Standardized Administration of the DMH/DD/SAS Frequency and Extent of Monitoring Tool (FEM) and Provider Monitoring Tool (PMT) for LMEs

Effective Date of Implementation of the Revised FEM and PMT -- April 1, 2010  

The FEM and PMT were developed to satisfy the requirements of Senate Bill 163 and 10A NCAC 27G.0608 to standardize the monitoring of providers. The purpose of the FEM is to assist the LME in determining and scheduling the frequency and extent of the monitoring of providers in their catchment area. The FEM is based on all information available to the provider. The FEM is a desk review and should reflect the provider's current status. The PMT is the standardized tool used by the LME for local monitoring after the FEM has been completed. The PMT is designed to efficiently assess provider performance in specific areas and to identify those areas that require additional follow-up. It is the intent of the FEM and the PMT to assist LMEs in directing staff resources where they are most needed for more in-depth or targeted monitoring.

The FEM and PMT have been revised in response to the current status of the MH/DD/SA system. The FEM was revised to allow more credit for national accreditation. The PMT has been streamlined and the organization and process by which the PMT is administered allows the LME to use components of the PMT rather than the entire tool. The backdrop upon which monitoring is conducted is when a review is indicated or required by circumstances or concerns.

Stakeholder groups consisting of Division staff, providers, representatives from the LMEs and representatives from consumer and family groups participated in the refinement of the FEM and PMT. Revisions were made based on this feedback.
