What IRS Form 1095-B Means to You

The Internal Revenue Service requires proof of the health insurance coverage you had in 2015. If you had NC Medicaid or Health Choice in 2015, the NC Department of Health and Human Services will mail you IRS Form 1095-B. This form will show each month you were covered by Medicaid or Health Choice last year. Look for Form 1095-B to arrive by March 31.

KEEP FORM 1095-B. The information on Form 1095-B may be helpful when you fill out your 2015 federal income tax return. Form 1095-B does not need to be mailed with your return.

IMPORTANT: The information on Form 1095-B has no effect on your current Medicaid eligibility or coverage.
