Report Fraud, Waste, Abuse, or Financial Mismanagement
This online hotline form, managed by the Office of the Internal Auditor (OIA), is designed for NCDHHS employees, vendors, grantees, or members of the public to report suspected fraud, waste, or abuse.
Do not use this site to report events that threaten safety, life, or property. If you require emergency assistance, please contact your local authorities.
Examples of matters that should be reported on this form:
- Falsification of NCDHHS official documents (timesheets, inventory, etc.)
- Falsification of official documents provided to NCDHHS from grantees or vendors
- Misuse or embezzlement of state or federal funds
- Improper billing or overcharging for goods or services
- Billing for services never rendered
- Financial Conflicts of Interest
Examples of matters that should not be reported on this form and should be reported elsewhere:
- General complaints about programs or services
- Human resources issues or employee grievances
- Equal Employment Opportunity (EEO) complaints (i.e. discrimination, harassment, retaliation)
- Child Welfare Services complaints
- Appeals of judicial decisions by State/Federal courts involving NCDHHS programs—Contact the court that issued the decision.
- Food and Nutrition Services (also known as Food Stamps) or Work First fraud—Please call 1-866-719-0141 to report.
- Medicaid fraud, waste, or abuse.
- Child and Adult Care Food Program fraud or the Supplemental Nutrition Program for Women, Infants and Children fraud—Please call 1-800-424-9121 to report.
Reporting Options and Your Privacy
Concerns may be reported anonymously, or you may choose to include contact information such as an email address or phone number so that we can follow up with you. If we cannot contact you, we may be unable to investigate.
The following statutes protect any individual filing a complaint:
- State Employees: State law requires that you report fraud, and the law protects you from retaliation. (G.S. §126-84;126-85)
- General Public: State law keeps the report you submit confidential, and the information will NOT be released to the general public. (G.S. §143-748)
In some cases, the information you submit may be referred to authorized representatives of the State or federal government, such as law enforcement. We will not voluntarily share your name with an individual or entity being investigated, but your identity could be revealed during an investigation due to the questions and inquiries needed to investigate the matter.
Reporting Your Concerns
Please remember to provide as much detail as possible about the alleged incident(s). Specific information such as names, titles/positions, location of the incident, dates, time(s), length of time that the problem has been occurring, and any documentation or evidence should be provided to aid in conducting a thorough investigation. Status reports are not provided for any investigations.
