Integrated Health Care Delivery Systems
Provides technical assistance to communities and safety net providers (such as public health departments, free clinics, rural health centers, hospitals, non-profit organizations, and local governments) seeking to improve access for vulnerable populations through integrated systems of care and innovative, strategic planning.
Supports the current initiatives and demonstrations with key internal and external partners:
- CHIPRA Quality Demonstration Grant
- Project Lazarus: Chronic Pain Initiative (CPI)
- Perinatal Quality Collaborative of North Carolina (PQCNC)
- Appalachian Care - Medical Mission
