- To obtain blank forms, how to use the new program and to submit a cost report or exemption form please read the updated instructions listed above. Again, no paper copies will be accepted, and the Office of the Controller has no access to enter information for providers.
- Agreed Upon Procedures are not required for facilities with six beds or less.

IMPORTANT – all AUP information and forms will have to be emailed by the due date (Sept. 30, 2023) to Susan Kesler or Yashaciqa Davis, see contact information below. If AUPs are not submitted the cost report submitted using the new program will still be considered incomplete.
