Working Title of the funding program: 2018 Rural Hospital Flexibility – EMS Sustainability Project
Purpose- Description of function of the program and reason why it was created:
The Rural Hospital Flexibility Program (Flex Program) is a federal grant program directed to State Offices of Rural Health to support:
• Improving the quality of health care provided in communities served by Critical Access Hospitals (CAHs)
• Improving the financial and operational performance of CAHs
• Developing collaborative regional and local delivery systems in CAH communities
• Facilitating the establishment of CAHs
The purpose of this project is to develop a community paramedicine program and leverage the work to improve patient’s health. The decrease in emergency department volume would indicate improved health.
Description- Brief statement of nature and scope of the program:
This competitive grant will be awarded to organizations that create a plan to perform one of the following activities:
(1) Complete education and training with local EMS partners
(2) Educate local healthcare providers on tracking/trending of patient data
(3) Arrange for local partners to collaborate on this project
(4) Develop a plan to sustain the program in the future
Funding Availability:
November 15, 2018 through August 31, 2019.
Successful grantees may receive up to $52,631 during the grant period. The maximum total grant award cannot exceed $52,631. All funding must be expended by August 31, 2019.

All applicants will need to complete the grant application, budget template, and contractor information sheet.
