NC System of Care Community Collaborative Map

North Carolina’s System of Care (NC SOC) Community Collaboratives are community-based groups comprised of family members, child-serving public agencies, and private providers. Collaboratives work with families, school systems, community-based organizations, and other stakeholders concerned with the behavioral health of all children in their community. Collaboratives bring together families and child representatives across diverse sectors to support coordinated service delivery and problem-solving.  

What do Community Collaboratives do? 

  • Support and oversee the development of their local System of Care.  
  • Bring people who care about children and families together.   
  • Promote teamwork and common goals, sharing resources, training and change in the community.  
  • Use local data to drive community projects and monitor progress.  
  • Keep discussion of cases/issues confidential.  
  • Communicate with NC Collaborative for Children, Youth and Families, other Local Collaboratives, the North Carolina Division of Child and Family Well-Being, and local Consumer and Family Advisory Committees.
  • Assess community needs and make plans to meet them.  
  • Inform the community about available services. 
  • Inform the community about how organizations contribute to the local collaborative.  
  • Ensure that youth and family members are included at every level of the System of Care.  
  • Ensure that evaluation activities occur and that outcomes inform decision-making.  
  • Create shared ownership by pooling resources.
