Independent Living Services

In partnership with their counselor, eligible clients will develop a plan for independent living that details the services and supports needed to achieve their goal. Services available include, but are not limited to the following:

IL program applicants receive an evaluation, counseling, guidance on eligibility requirements and referral to community agencies. This includes:

  • Medical, physical and psychological exams and evaluations.
  • Interpreter services.
  • Driver evaluation.
  • Visual and hearing exams.

Guidance, counseling and service coordination are an integral component of the independent living rehabilitation process that enables individuals to live and interact within their community of choice. Examples of guidance and counseling services: 

  • Helping client understand their diagnosis, impairment and functional limitations.
  • Assisting client adjust to the emotional issues surrounding their disability.
  • Facilitating treatment by coordinating with medical providers.
  • Exploring the strengths, interests and abilities of the individual.

Teaching skills that enhance the ability of a client to live independently, like budgeting, cooking, transportation and more. IL skills training may be provided individually or in group settings, and could include:

  • How to manage money and other financial skills that support informed decision-making.
  • Training on how to operate a microwave, turn on a stove, boil water and prepare simple meals.
  • Learning how to access public transportation, including the bus routes and paratransit

A qualified professional (such as a registered nurse, occupational therapist or physical therapist) may recommend this service based on the independent living skills and needs of the consumer. In partnership with their IL counselor, clients plan for part-time personal assistance to facilitate activities of daily living, including access to the community. The consumer hires, fires, pays and performs all duties of the employer, including paying required state and federal taxes. The consumer is reimbursed for the service provided by the personal assistant worker.

These services help an individual with disabilities make a transition from an institutional setting back into the home and community. Consumers use these services after completing an individualized plan for independent living and as the final phase in returning to the community. These services may include:

  • Payment for rent and utilities.
  • Rent and utility deposits.
  • Basic furniture and small appliances.
  • Recreational therapy.
  • Moving expenses.
  • Transportation and vehicle expenses.

To help individuals with disabilities live independently in their homes and access services in the community, the IL program may recommend rehabilitation and assistive technologies and services. These services are coordinated with other providers and comparable benefits, such as Medicaid, Medicare and private insurance, and can include:

  • Home modifications (ramps, accessible bathrooms, widening of doors and entrances).
  • Wheelchairs.
  • Communication devices.
  • Vehicle modifications.
