Why did I get more than one Form 1095-B?
It is possible to receive more than one Form 1095-B if you were covered by more than one health insurance provider during 2015. For example, if you had private insurance for seven months, and Medicaid or Health Choice for five months, you may receive two forms.

Is it possible to receive a second Form 1095 from the same provider?
Yes. Certain changes or corrections in your information can trigger the mailing of a second form to replace the one you received earlier.

My Form 1095-B lists family members who no longer live with me. How do they request a separate form?
You or a family member can ask for a separate form and have it mailed to a different address by calling the Division of Medical Assistance Call Center at (888) 245-0179 or (919) 855-4780.

I moved and want to be sure my 1095-B Form is mailed to my new address. How do I change my address?
Your Form 1095-B will be mailed to the address on file with your county Department of Social Services (DSS) office. Contact your case worker at your DSS office to verify your address and ask for another form to be mailed to you.

I did not receive a Form 1095-B. How can I get my form?
Forms will be mailed by the first week of March 2016. If you haven't received your form by the end of March, contact your case worker at your county Department of Social Services office to verify that your address is correct and to ask for another 1095-B to be mailed to you.

I received a Form 1095-C. How is that different from Form 1095-B?
IRS Form 1095-C is used by employers with at least 50 full-time workers to report employer health plan coverage. IRS Form 1095-B, on the other hand, is mailed by the NC Department of Health and Human Services to report Medicaid or Health Choice health insurance coverage.
