Community MH/SUS Client Surveys

2024 Survey Administration Period (Aug. 12, 2024 – Sept. 20, 2024)

Providers of community mental health and substance use disorder services who are participating in the annual North Carolina Perceptions of Care Survey may access the surveys using the links below. Please select the correct survey-Adult, Youth, or Child Family- for the age of the client. Spanish language versions of each survey are available.

Please refer to Survey Administration Guidelines provided by your Tailored Plan Survey Coordinator. Please contact your Tailored Plan Survey Coordinator with any questions about administering the surveys.

Providers are encouraged to invite every client who receives a Tailored Plan service during the survey administration period to complete the survey. If they have not been discharged from services, clients who received an LME/MCO/TP service in June, July or August 2024 may also be surveyed, even if they do not receive a service during the survey period. Surveys may be administered in-person or by use telephonic or two-way audio-video methods.

Web-based Surveys:

Providers must click the "submit" button at the end of the survey to save and submit responses. There is no auto-save function.

Paper Surveys:

Providers may also offer paper copies and submit responses electronically. Responses for all surveys must be submitted electronically by September 20th.
