Inclusion Connects Reports

The Inclusion Connect report provides progress updates on North Carolina Department of Health and Human Services (NC DHHS) initiatives to enhance service access and support for individuals with intellectual and developmental disabilities (I/DD). It meets the reporting requirements set by the court in the Samantha R. case against NC DHHS and the State of North Carolina. NC DHHS uses these reports to inform future efforts to improve its services.

We value accurate data. We collaborate with local management entities/managed care organizations (LME/MCO) and NC DHHS's systems and tools to continuously improve data collection and reporting. To protect privacy, NC DHHS does not report totals of fewer than 11 individuals.

NC DHHS will publish the Inclusion Connects Workplan in February 2025. This document will provide further details about the data and actions taken to meet the needs of North Carolina’s I/DD community.

Inclusion Connects Report December 5, 2024 - The December 5, 2024 report is based on data gathered before implementing the new Tailored Plans, which marked a significant shift in managing whole-person health, including I/DD, physical health, and pharmaceutical services. As this transition represents a change in how services are delivered and data is reported, comparisons with subsequent reports should be made cautiously.
